The PACH Conference 2026, Where Practice Meets Progress, Communities Drive Change, will bring together Community Health Workers (CHWs), Community Care Hubs, policymakers, public health, funders, and industry partners for two days of dialogue, learning, and collaboration. This year’s gathering will highlight how CHWs and Hubs are turning on-the-ground practice into real progress—expanding partnerships, shaping workforce identity, and adapting to change with advocacy at the center. Through breakout sessions, keynote conversations, and policy panels, participants will explore how communities are not just responding to challenges, but actively driving solutions that shape systems.
In addition to engaging sessions, the conference offers multiple opportunities to connect. Networking breaks, coalition-building spaces, and interactive vendor tables will create time for relationships to deepen and new partnerships to emerge.
