Impact and Achievements (2018-2023)

Between January 1, 2018, and October 31, 2023, Ohio's 12 certified Pathways Community HUBs have made a significant impact across the state:

Total Clients Served: 47,404
The number of clients served increased steadily from 4,978 in 2018 to 11,740 in 2023, reflecting the expansion of HUB services and their growing reach.

Total Pathways Completed: 327,076
HUBs completed an impressive 327,076 pathways during this period, addressing social determinants of health and guiding clients to essential services.

Social Determinants of Health Barriers: 86,377 barriers addressed
CHWs have worked to remove barriers to health by helping clients access food, housing, and other critical resources.

Clinical Care Connections: 67,534 connections
HUBs facilitated connections to clinical care, ensuring clients received necessary medical services to improve their health.

Client Education and Support: 173,165 services provided
HUBs also provided extensive education and support to clients, empowering them to make informed health decisions and promoting self-sufficiency.
