This form is for organizations requesting a table to share information, present a poster, or engage attendees about programs, services, or products. Submission of this form does not guarantee a table until confirmation and payment are received.

• Tables are intended for information-sharing, learning, and engagement with conference attendees.
• PACH reserves the right to decline applications that do not align with the mission or audience of the conference.
• Vendors and presenters are responsible for their own materials, setup, and breakdown.
• No refunds will be issued after 3/31/26.

Photos and/or video is being taken at this event and may appear in Partnership for Advancing Community Health(PACH) marketing materials. By attending this event, you hereby authorize PACH to use your image in any and all use related to the educational mission of the university without compensation. Please notify the department hosting the event if you wish to attend but not have your image used in this way.
By registering for the conference, you agree that PACH may share your name and contact information with event sponsors so they can follow up with you after the event.
